Please note that in order to get credit for completing a module's handouts, you must turn them in completed (if needed) plus signed & dated. Included in the forms that you will need to turn in are the Summary Forms.

The following links comprise the documents that are needed in order to successfully complete the MWRS DUII program.
Information
Please know that the program summaries and the Oregon Health Authority DUII pre & post test will not be posted onto this website. These forms can be received through email and/or United States Postal Service. You may also come into the office, provided that you wear a mask at all times, and complete a temperature check.
